Bounce’s Home Care and Rehabilitation program allows you to receive quality and guided healthcare services in the safety, privacy and comfort of your own home.

We ensure that as an alternative to inpatient rehabilitation that as our patient you will:

achieve your expected goals and outcomes
regain your independence
improve your quality of life

We also provide home care to patients not returning home from hospital who would simply like to see a clinician at home rather than attend a clinic.

If you have a condition that requires treatment, we can provide appropriate care in the comfort of your home.

Home Rehabilitation Coordination

Each patient has individual needs that are managed by our Care Coordinators who ensure your transition from hospital to home is as smooth as possible. Our Care Coordinators work with our team of clinicians to ensure your treatment and rehabilitation meets all your needs, goals & expectations.

Programs vary between 4-6 weeks depending on your needs. Your program will be discussed with the hospital staff at your time of discharge.

Referral Process

We require a referral from a doctor or from the hospital prior to commencing your home care. Then we liaise with your health fund and once this has been accepted, your care coordinator will contact you to structure your home based program.

If you are not a hospital discharge patient and simply require a physiotherapist to attend your home for treatment, no referral is required.
